Home and Community-Based Services (HCBS)
Home and Community-based Services are person-centered and delivered in the home and community. These Medicaid Programs provide service funding & individualized supports that allow for more choice about how and where services are received. We believe that every person should live as independently as possible in their own home. Staff help clients maintain daily living skills, learn new skills, and increase involvement in their community of choice. HCBS services are based on an individual’s needs. Support is provided on an hourly basis up to a 24 hours each day within the individual’s own home.
Inspiring Lives provides HCBS services for the Intellectual Disability, Elderly, AIDS/HIV, Physical Disability and Brain Injury waiver programs. Supported Community Living (SCL) offered under these waivers (except Elderly Waiver) focuses on the individual maintaining and building on daily living skills within the home as well as in the community. Consumer Directed Attendant Care (CDAC) services under the waivers help people do things that they normally would do for themselves if they were able. People may need help to remain in their own homes. Assistance is provided as needed to complete daily living tasks. Inspiring Lives also offers Respite services, providing temporary, substitute supports or living arrangements for cared-for individuals in order to provide primary caregivers a much needed break.
Habilitation
The Habilitation waiver is for individuals with the functional impairments associated with chronic mental illnesses. Habilitation Services are designed to assist participants in acquiring, retaining & improving the self-help, socialization & adaptive skills necessary to reside successfully in home and community-based settings. An individual practices skills with staff support to better manage their mental and physical health while living in the community.
